To claim your card, go to www.ohaus.com/exrebate, fill out the form, and attach a copy of your receipt. Note that this offer is valid in the US and Canada only. Recipients in the US will receive a Visa Reward Card, and those in Canada will receive an online credit to redeem gift cards for a variety of stores.
